Home and Community Care Support Services Mississauga Halton helps to coordinate the delivery of government funded care in the home. This may include:

Visiting Hospice provides support in the community to those living with a life limiting illness, and their families.

Residential Hospice offers a place to receive quality end-of-life care for those who for whatever reason, cannot receive enough support in their own homes. It helps provide a homelike environment for patients and their family in the final days and weeks of life, when the goals are for comfort care.

Palliative Care supports are available across the hospital system. In most cases, palliative care needs are met by the usual care team in the hospital clinics and inpatient units. In some cases, a specialist team may become involved and sometimes, a specialized clinic or unit may be accessed.
Halton Healthcare has three sites. There is a specialty outpatient Palliative Care Clinic at the Oakville site as well as inpatient consultation service. There are dedicated palliative care beds available at all hospital sites.

Trillium Health Partners has three sites. There is a specialty outpatient Palliative Care Oncology Clinic at the Credit Valley and Queensway sites for cancer patients, as well as inpatient consultation services at the Mississauga Hospital and Credit Valley Hospital sites. There is a Palliative Care Unit at Credit Valley site an and McCall Centre (Queensway Site).

Long term care homes offer restorative as well as palliative care services to individuals no longer able to receive enough care in their own homes. The Nurse Practitioners Supporting Teams Averting Transfers (NPSTAT) as well as the Palliative Pain & Symptom Management Consultants offer support to manage symptoms as needed.
